Collingswood Nursing and Rehabilitation Center's social services team provides continued assessment and support of residents' emotional and psychosocial needs, as well as counseling for advance directives and end of life decisions.
The social workers coordinate quarterly care planning conferences, which allow
residents, families, and staff to develop, address and manage plans for every
resident on an ongoing basis. This interdisciplinary approach assures there
is an open dialog between, families, loved ones and the care team during their
stay.
